Independent Living Skills
Our purpose is to furnish independent living services on a 1:1 ratio to adults over 18 who qualify for regional center services based on the diagnosis of developmental disabilities. The services will be determined based on what is needed by the individual, their IPP, PCP or Self-Determination Program and by the ACRC Service Coordinator in conjunction with the Purchase of Service authorization. Together as a team, TRAILS along with ACRC Service Coordinator, the individual receiving services and any family members involved, will create a service plan based on the individuals IPP and goals. The curriculum will be in compliance with T17 and TRAILS vendor service code standards.
These services provide education and training on a variety of necessary skills to maintain an independent life style and may include, but are not limited to: cooking, cleaning, health and medical appointments, public transportation, banking, bill paying, grocery shopping, etc.
Our goal is to provide these services in such a manner as to be positive, supportive, instructive, constructive and encouraging of the individuals needs and personal goals. With our assistance, we will help those we serve to live a safe, independent life in their community and be successful at maintaining their level of independence once it is reached.
